第一屆國際奧林匹克數學競賽是於羅馬尼亞舉行的。其後除了在1980年因蒙古內亂而被停辦外，每年都會舉辦国际奥林匹克数学竞赛[1]。由於該競賽原為參與華沙條約組織的東歐國家，於東方集團的影響下，[2]早期的競賽都是在東歐國家舉行的，漸漸地擴展至其他國家。[3]不同來源指出了早期不同的舉辦城市及舉辦日期。[4]

第一屆的競賽中有七個國家參與——包括保加利亞、捷克斯洛伐克、東德、匈牙利、波蘭、羅馬尼亞及蘇聯，以主辦國為首名。[5]參與國家的數目自此提升了：1969年時有14個，1989時有50個，2009年時有104個。[6]

北韓是惟一一個國家被發現作弊，導致於1991年的第32屆競賽及2010年第51屆競賽被取消資格。[7]於2011年1月，谷歌捐贈了1億歐元給国际奥林匹克数学協會以資助2011年至2015年的競賽。[8]
